This article describes a service-learning experience developed and implemented by strengthening an existing partnership among a baccalaureate school of nursing, a federally qualified community primary health care clinic, and senior baccalaureate nursing students. Following participation in team-building experiences at a Partners in Caring and Community Institute, representatives of the three partners collaboratively designed a service-learning experience for senior undergraduate nursing students and agency professional staff. This partnership enabled clients with diabetes from the clinic to receive enhanced self-management support.
